Duty hours: Duty hours: between 9 AM – 5 PM EST (Part-time: 20 hours per week)
Salary Offer per case: $150 - $200

What are we looking for?

JOB POSITION: Care Manager

We're looking for a compassionate Care Manager to develop individualized patient care plans. Care Managers are primarily focused on overseeing and improving client and family care and health outcomes.
The Care Manager will be responsible for the assessment, reassessment and coordination of care and services, including ongoing monitoring of appropriate and effective person-centered care plan, member education.
He or she will regularly communicate with the people involved in the client circles i.e.: PCP, School and education workers and other providers and integrate the member, caregiver and other provider feedback into the assessment and planning.
To ensure success, you will guide patients through the healthcare system and cultivate relationships with healthcare providers. Top candidates can multitask in a fast-paced environment and negotiate effectively.

RESPONSIBILITIES:
• Document all case activity, client progress, including outreach, consent development and assessment.
• Complete CANS, UAS and all other necessary screenings and training, as per the Health Home requirements.
• Ensure continuity of care for newly enrolled members.
• Identify and prioritize the client’s need and preferences and promote the member’s ability to self-manage in the greatest extent possible.
• Manage case load and serve as the primary point of member contact and ensure effective communications among members, caregivers, providers, and community support.
• Maintain timely, complete, and accurate documentation on the GSI system as services and support are being provided and completed.
• Completing a face-to-face monthly wellness check with the client and documenting it properly.
• Is willing to do whatever necessary training required as well as get proper support from the supervisor overseeing the Care Manager. training will require some in-person training as well as online trainings.
• Participate in supervision.

QUALIFICATIONS:
• Bachelor's degree in social sciences, social work, nursing, or a related field.
• 1 - 2 years of work experience in public health or related field.
• Demonstrate passion and commitment to excellence
• Understands and adapts appropriately to issues related to communication, cognitive and other barriers.
• Ability to prioritize and follow through on tasks needed for the client and family.
• Possess basic computer skills and demonstrate a willingness to learn more advanced skills.
• Proficient with Healthcare Management Systems and Microsoft Office Word and Excel.
